New Hire Standard Computer Request

Use this form for newly hired full-time employees.

• Existing employees should use the New Standard Computer form or the New Non-Standard Computer form.
• New and existing part-time employees should use the Refurbished Computer (SORT) form.

Configuration Options for Standard Computers:

All computers will be equipped with the appropriate version of Microsoft Office, MS Anti-Virus, and other standard software. The system will be part of the Unified Desktop Management program to provide automatic upgrades of anti-virus software and Windows patches. These configurations are subject to change contingent upon funding.
